Wisdom Tooth Pain: Causes, Remedies For Relief and Treatments

Wisdom tooth pain whoever has felt understands how difficult it is. Sometimes the cheeks are swollen, it is difficult to open the mouth and there is severe pain. The wisdom tooth is the third molar. It is the last adult tooth to erupt. All teeth go up within the ages of 12 and 13, but wisdom teeth go up between the ages of 17 and 25. It is a common missing tooth, not erupt for all. 

In many cases there is no pain, the wisdom tooth is easily erupting. In those cases, the tooth grows slowly. It takes months or years to erupt. And patient start to feel mild pain. 1. Pericoronitis:

Pericoronitis is inflammation of coronal soft tissues surrounding a partially erupted tooth. It usually affects the lower third molar i.e wisdom tooth. And most of the time the wisdom tooth pain is due to pericoronitis. When the crown portion of the tooth erupts partially it injures the soft gingiva and operculum resulting in pain. In the case of wisdom tooth most of the time this is difficult to brush properly and remove food particles.

It results in infection, which aggravates the pain condition. As a result of inflammation swelling and redness, gingival bleeding starts of the occur. Due to uncleaned foodstuff, bad breath produces. If mechanical trauma from the opposite tooth occurs at the flap it worsens the condition.  

In the advanced stage bad breath from pus, difficulties to open the mouth (trismus), difficulties in swallowing ( dysphagia), generalized illness i.e fever, loss of apatite, and cervical lymphadenopathy may occur. This infection can spread to the cheeks and neck or facial plane.

The impacted wisdom tooth is a disorder when it is prevented from erupting in the mouth. According to angulation impacted teeth can be classified as vertical, horizontal, distal, and mesial impaction. Extreme excruciating pain in the back of the mouth which is localized usually, headaches, earaches, swollen neck, and shoulder glands are the sign and symptoms of impacted wisdom tooth.

An impacted tooth can overlap nerves, damage nearby soft tissues, teeth, and bones. So many dentists recommend surgical removal of teeth. Impacted wisdom teeth removal is difficult to remove. 4. Cyst development:

A follicular cyst may develop in unerupted wisdom teeth. A  tooth develops in a sac within the jawbone. when the sac is filled with fluid, it results in a cyst that can damage the jawbone, teeth, and nerves. a benign tumor may also develop, but it is very rare

5. Caries and pulpitis: 

Food impaction is very common around the wisdom tooth. Because of its narrow position food lodges easily and it is difficult to remove. It causes the development of caries easily. Most of the time it remains unnoticed by the patient due to bad access. Further pulpitis develops and causes severe pulpal painUsually, dentists suggest root canal treatment for pulpitis teeth and try to save the teeth. 

But when it is about wisdom tooth pain due to pulpitis, the treatment plan mostly goes for extraction. The wisdom tooth usually has very bad access. Bad access tooth is difficult for both patient and the operator as root canal treatment is very technique sensitive.

Wisdom tooth pain relief:

  1. Pain killers: Ibuprofen is best for pain relief at home.  Paracetamol/ acetaminophen may also reduce pain.
  2. Cold compression: The use of an icepack externally may give some comfort.
  3. Hot salt water rinsing may give some relief in wisdom tooth pain as it is a natural disinfectant. It also helps to wash away food particles. 6/7 times rinsing of hot water will be most beneficial. It also helps with normal blood circulation to reduce infection and wisdom tooth pain.
  4. Chlorhexidine Mouthwash or hydrogen peroxide rinsing is useful sometimes in decreasing pain.
  5. Chewing of cloves and place them close to the decayed tooth will help to reduce pain as the clove is full of eugenol i.e. natural anesthetic.
  6. Similarly chewing of garlic and place them in the inflammatory site may give comfort in severe pain as garlic has anti-inflammatory effects.
  7. Placement of used tea bags in the inflammatory area will help to reduce swelling and helps in wisdom tooth pain relief.
  8. Try to keep the area clean. Try to do toothbrushing whatever severe maybe the pain is. Food impaction will aggravate the pain.
  9. Practice slowly to increase mouth opening. It will help during dental treatment.
  10. Never ever give any hot compression externally. It increases swelling.

Wisdom tooth pain treatment:

Wisdom tooth pain treatment plan should be done after proper investigation. In that case, an x-ray like OPG ( orthopantomogram) is the best option. Proper clinical examination and different investigation will guide the treatment plan. The dentist may suggest antibiotic like amoxicillin or erythromycin if systemic complication like fever, lymphadenopathy present

Operculectomy:

If wisdom tooth pain is related to pericoronitis then operculectomy ( surgical removal of operculum) may be an option. But before planning operculectomy space available for the wisdom tooth and angulation should be revied properly.

Removal of tooth:

Wisdom tooth removal is commonly suggested in case of pericoronitis, impacted wisdom tooth, or pulpitis in wisdom tooth. As this tooth not help in our mastication, bad access, and recurrent pericoronitis after operculectomy ; other treatment options are not usually encouraged. . Simple extraction and surgical extraction both are the option of wisdom tooth removal. Planning of wisdom tooth removal depends on its angulation and degree of impaction. The surgical approach is a must in case of impacted wisdom teeth.

Prevention of wisdom tooth pain:

wisdom tooth that simply comes to its normal position without disturbance also needs attention. So regular check-up at a 6-month interval to the dentist is the best option of prevention. Maintenance of proper oral hygiene by regular tooth brushing and hot salt water mouth rinse are able to prevent many complications.
